6. Duly filled Application Form.

7. Photo (Passport Size) - 1 No.

Pre-requisite :

1. Open Well / Bore Well (attested in Adangal by VAO)

2. Motor Pump - Specifications & Invoice

Process :

1. Submit the documents along with application form at the Executive / Assistant Engineer (Operation & Maintenance Office) of the jurisdiction.

2. For registering the application Rs.50/- has to be paid.

3. After registering the application an acknowledgement mentioning the registration number and date of registration will be given to the applicant.
